24-Hour Home Care in Los Angeles, CA

When someone can no longer be left alone, part-time help stops being enough. Urban Health Care Solutions provides 24 hour home care across Los Angeles using rotating teams of trained caregivers, so there is always someone awake, alert, and in the room. Your loved one stays in their own bed, in their own home, with support that never clocks out.

The Night You Realized They Can't Be Alone Anymore

For most families, there is a specific moment. A fall in the bathroom at 3 a.m. A pot left on the stove. A front door found standing open. A phone call from a neighbor. Or a hospital discharge nurse saying the words "they'll need someone with them around the clock" while you are still trying to process everything else. Up until that point, you were managing. A few hours of help here, a daughter stopping by after work there, a neighbor keeping an eye out. Then something happens, and the gaps in that schedule stop being inconvenient and start being dangerous.

You are probably weighing two options right now: move them into a facility, or figure out how to keep them home safely. Most families we meet want the second one. They just don't know it's actually possible.

It is. Round the clock care at home is what we do.

What Is 24 Hour Home Care?

24 hour home care means a trained caregiver is present in the home at all times, day and night, with no unsupervised gaps. Care is delivered by a rotating team, typically two or three caregivers working scheduled shifts, so that the caregiver on duty is fully awake and attentive for their entire shift.

That last part matters more than most families realize when they start comparing options.

24 hour rotating shift care (what we provide)

Caregivers work in scheduled shifts and hand off to the next caregiver. Every caregiver on shift is awake and on duty the whole time. There is no expectation of sleep, no need for a spare bedroom, and no drop in attentiveness overnight.

Live-in care

One caregiver lives in the home and is granted a sleeping period and break time under California labor rules. It costs less, but there is a tradeoff: overnight coverage is not truly continuous, and the arrangement requires private sleeping accommodations.

Signs Your Family May Need Around the Clock Care

Not everyone who needs help needs 24 hour support. These are the situations where families usually do:
Nighttime falls icon

Nighttime falls or unsteady bathroom trips.

Most falls happen between the bed and the bathroom, in the dark, with no one there.

Wandering prevention icon

Wandering or exit-seeking

Someone with dementia who has left the house once will very likely try again.

Sundowning behavior icon

Sundowning

Confusion, agitation, and restlessness that get worse in the late afternoon and evening, right when family caregivers are most exhausted.

Hospital discharge care icon

A recent hospital or rehab discharge

The first 30 days home carry the highest risk of a readmission, and that is exactly when supervision matters most.

Medication timing icon

Medication timing across the full day

Doses at 6 a.m. and 10 p.m. only work if someone is there to remind them.

Caregiver burnout icon

Family caregiver burnout

If a spouse or adult child has stopped sleeping through the night, the safety risk now includes two people, not one.

End of life home care icon

Advanced illness or end of life at home

Continuous comfort and presence, alongside a hospice team if one is involved.

Once is a warning. Families rarely regret acting on it.

    Free in-home assessment

We come to the home, meet your loved one, look at the space with fall risk in mind, and build a shift schedule around their real routine. We'll be honest with you about what level of care the situation actually calls for.

    Meet your care team

We match caregivers based on the care needed, personality, and language, then introduce the team before the first overnight shift so no one is meeting a stranger in the dark. Care can typically begin within a few days, and faster for urgent discharges.
